Vendors and contractors
IRS Form 990 Part VII-B lists only the five highest-paid independent contractors at or above $100,000. A vendor below that threshold does not appear — absence here means 'not in the top five', never 'no vendor'.
| Vendor | Service | Location | Amount |
|---|---|---|---|
| MDHY LLC | MEDICAL SERVICES | MOORESVILLE, NC | $6,568,252 |
| FRIEDOMTECH LLC | TECHNOLOGY SERVICE | HOWELL, NJ | $622,249 |

| ISTA PERSONNEL SOLUTIONS LLC | CONTRACTED STAFFING | HOWELL, NJ | $417,562 |
| ADEST LAW PLLC | LEGAL SERVICES | CEDARHURST, NY | $330,000 |
26 contractor records across 6 filing years — vendor switches and tenure are computed from this history.

Statement of Condition While performing our audit, we noted that the Center did not properly determine the sliding fee discount category given to certain patients selected for testing based on the sliding fee scale in effect for the year ended December 31, 2023. Cause The condition can be attributed to human error and the lack of internal controls to review and ensure that the proper…
